My Allianz Moment Contest (Malaysia)

Contest Period :
1st October till 31st October 2013

How To Participate :
- Create a video titled “My Allianz Moment ….. (describe the video)”. The video must be 30 seconds (or less) and must contain the word “Allianz” mentioned at least once. Videos not fulfilling these criteria would be disqualified.
- Upload the video on Youtube.
- E-mail to online.feedback@allianz.com.my with the following details:
a. The video link
b. Indicate Category A or Category B:
c. Full Name and IC Number or Passport Number (for Category A)
d. Full Name and Agent Code/Employee ID (for Category B)
e. Contact details and Mailing Address

Prizes :
1st Prize : iPad Mini
2nd Prize : iPod Touch
3rd Prize : iPod Nano
4th to 6th Consolation Prize : 10 Movie Vouchers (Each Winners)
